Insurance Reimbursement How-To

  • HOW TO GET REIMBURSEMENT FROM YOUR PRIVATE INSURANCE for DPC = Direct Primary Care

    • !!! DON'T do this with MEDICARE/SOONERCARE/TRICARE, else you'll be committing fraud !!!

      • look up online via Google for "reimbursement forms for patients" + "your insurance company name"

      • find the appropriate form most will be in PDFs, make sure it said somewhere on top Reimbursement "for Patients" or "for subscribed/insured person" on it

      • make sure you carefully distinguish between "insured/subscriber" vs. "patient". For example, "patient" = you who was seen and treated by me directly, while "insured/subscriber" = your spouse or the purchaser of insurance who pay the insurance bills

      • make sure you DON'T check any boxes that say you want them to pay directly to the "Provider" (meaning me your doctor) because I won't cash that check if they send it to me

      • attach to it the INVOICE (the one with our logo and "paid in full" stamp and ICD-10 codings typed on it, see sample image below) that we usually give you at the end of your visit whenever we collected payment from you

      • follow instruction on the form closely, fill out everything--unless the form told you it's optional

      • mail them all in

          • sometimes they have options to submit everything online too, so if you prefer electronic filing, you should look for that option

      • then wait for a check from your insurance company written with YOUR NAME on it as the receiver. Enjoy!
